There is a boss found underwater in the 
Natural Aquarium (here 
Natural Aquarium #1213) called 
Seana.
It is recommended to have the
Water Orb to move freely underwater. But if you manage to get to her and defeater her without the Water Orb, you will get this achievement.
This one goes well with a 0% item run (see below).
